Skull Fractures

Skull fractures: Intrusive injuries by definition involve fractures. Closed head injuries can also cause skull fractures, which could be linear, depressed, or comminuted. The presence of a fracture implies that a huge amount of force was involved in the injury due to the accident. However, most patients with simple linear fractures and no neurologic damage are not at high risk of brain injuries. Fractures that involve special circumstance risks include the following:

  • Fractures in patients with neurologic damage: Such patients are at a much higher risk of intracranial hematomas.
  • Depressed fractures: These fractures tend to have the highest risk of tearing the dura, possibly damaging the underlying brain, or both.
  • Temporal bone fractures that cross the area of the middle meningeal artery: In these fractures, developing an epidural hematoma is a great risk.
  • Fractures that cross one of the major dural sinuses: These fractures may cause considerable hemorrhage and venous epidural or venous subdural hematoma. Damaged venous sinuses may cause later thrombose and cerebral infarction.
  • Fractures that involve the carotid canal: These fractures may result in a carotid artery dissection.
  • Fractures of the occipital bone and base of the skull (basilar bones): These bones are thick and strong, so fractures in these areas reveal a high intensity impact and a meaningfully increased risk of brain injury.
  • Basilar skull fractures that extend into the petrous part of the temporal bone can often hard middle and inner ear structures and can possibly impair facial, acoustic, and vestibular nerve function.
  • Fractures in infants: The meninges may become embedded in a linear skull fracture with subsequent development of a leptomeningeal cyst and expansion of the original fracture ("a growing fracture").
